Navigating the healthcare system and evaluating options can be very time consuming and confusing when you don’t know the right questions to ask.

We are experts at the language and the process. We’ll assist you in researching your options to get the help that is needed. At the same time, we are there to support you through the emotional journey that often accompanies caring for an aging parent. We take the mystery out of Eldercare Planning and ensure that you are not alone when trying to be there for the person you love, offering as much or as little assistance as you need.

Initial Family or Advocate Consultation:
A CareSmart Seniors Consultant is ready to listen to your concerns. We offer a free consultation by phone as an introduction to addressing your needs and concerns.  This can lay the groundwork for how to proceed in your Eldercare role and how to best support the senior you care for.

A Circle of Care Consultation:

The ElderCare Consultant meets with the older adult in their own home to do a comprehensive in-home evaluation of their well being and safety. This may include assessment of daily challenges or limitations, formal and informal support systems, home safety and social functioning. Significant others (spouses, family members) are welcome to attend this interview. This in-home meeting usually takes 1.5 to 2.5 hours with the client.

The Consultant will then complete a written “Wellness Plan” that documents the information gathered and identifies areas of concern, the expressed goals of the older adult and family members and presents potential solutions. We will provide recommendations that include publicly available or informal services whenever possible, as well as paid services where indicated.

A follow up review of this “Roadmap for Care” will be done to discuss an implementation plan of the recommendations. It will be left to the discretion of the senior and their support network to decide which areas of the plan, if any, will require our assistance.

Ongoing ElderWell Senior Support Services:

Once a Circle of Care Consultation is completed, CareSmart Seniors Consulting staff are available as a resource to assist in ongoing support of the client.

Scheduled Wellness Visits can be established on a weekly, bi-weekly or monthly basis. Sporadic or emergency assistance can be provided according to staff availability. Follow up reporting keeps family members connected and up to date via email, phone or family website portal as indicated and preferred.

Our services include:

    • Liaising with support agency staff and coordinating involved service providers to ensure a collaborative approach to a senior’s care is achieved.
    • Attending medical appointments and provision of follow up reporting to the family
    • Life management assistance
    • Health and fall prevention strategies for optimum wellness
    • Ensuring sociability and planning to keep the client engaged
    • Observing and reporting noted changes in the well being of the client
